If you live in
Adams, Arapahoe, Broomfield,
Denver, Douglas and Jefferson county in Colorado you
will may enroll in the Medicare
Advantage HMO plans from
Anthem Blue Cross and Blue Shield of
Colorado with or without prescription drug coverage called
Medicare Advantage Plans.
If you live in Alamosa,
Archuleta, Chaffee, Costilla, Denver, Dolores, Douglas, El Paso,
Elbert, Fremont, Gilpin, Gunnison, Hinsdale, Jefferson, Kiowa,
Larimer, Las Animas, Lincoln, Mineral, Otero, Park, Phillips,
Rio Grande, Routt, Saguache, San Juan, San Miguel, Teller and Weld,
you are eligible to enroll in the
Smart Value Plus Medicare
Advantage Plan (PFFS) or the
SmartValue Classic Plan (PFFS)
with Part D prescription benefits.
Anthem Medicare Advantage members can choose from a network
of doctors, specialists and hospitals for their health care needs.
You may use any doctor who is part of the network. You may also
used doctors outside of the network but you may have to pay more
for the services you receive.
All Medicare Advantage Senior Plans agree to stay in the
program for a full year at a time. Each year, the plans decide
whether to continue for another year. Even if a plan decides not
to continue, you will not lose Medicare coverage.
SureValue is an HMO plan
with a Medicare Advantage
contract. Anyone who resides in the service area and who has
Medicare Parts A and B may apply. This healthcare plan offers
valuable benefits – coverage you can count on – at the
doctor and dentist, plus pharmacy discounts.
A Medicare Advantage Private Fee–for–Service (PFFS) plan
works differently than a Medicare insurance supplement plan.
Your doctor or hospital is not required to agree to accept the
plan’s terms and conditions, and thus may choose not to treat
you, with the exception of emergencies. If your doctor or
hospital does not agree to accept our payment terms and
conditions, they may choose not to provide health care services
to you, except in emergencies.
