If you live in Cocino County Arizona, you are eligible to purchase the Desert Canyon Community Care Plus HMO Medicare Plan. If you live in Mohave or Yavpai Counties, you have the choice of purchasing the Plus Plan (HMO) and Plus Point (HMO/HMO POS). Your Part D coverage for prescription drugs is included in these plans.

All Medicare Advantage Plans agree to stay in the program for a full year at a time. Each year, the plans decide whether to continue for another year. Even if a plan decides not to continue, you will not lose Medicare coverage.

Desert Canyon Community Care Plus and Plus Point plans are HMO or HMO POS plans with a Medicare Advantage contract. Anyone who resides in the service area and who has Medicare Parts A and B may apply.
